Who touched what, and exactly when.
The audit log records every authenticated request, append only, in line with HIPAA 164.312(b): who, what action, which resource, status and severity. Accesses to protected health information are tracked separately, and you can filter by action, resource, severity or user whenever a question comes up.
Session timeouts and per role scoping round out the picture: practices are scoped to their practice, laboratories to their lab, and partner labs to the cases referred to them.
